$100 per month OR $1,000 if paid in full upfront (20% discount), entitles you to the following benefits:

Service Terms

• All included services rendered in-office.
• Appointment required.
• We reserve the right to refuse membership to anyone at our discretion.
• We reserve the right to change the terms of this membership at any time without notice.
• Rewards may not be redeemed for cash.
• Membership card benefits are only valid for the cardholder and may not be transferred.
• No pre-purchase of future treatments prior to expiration of program.
• Treatments must be completed prior to expiration of enrollment year.
• Stoker Aesthetics reserves the right to terminate or modify the program at any time.

Cancellation Terms:

• Cancel in writing
• 60-Day Notice to Cancel
• 12-month minimum commitment
• Fee Charged in cancelled during first 6 months
• No refunds of paid membership fees or unused treatments. A cancellation fee of $200 or more may apply if the agreement is terminated early.
